au d\u00e9veloppement et au maintient des attitudes favorables \u00e0 sa sant\u00e9<br \/>&#8211; \u00e0 une meilleure prise en charge de sa maladie hypertensive.<\/p><p><span style=\"font-style: inherit; font-weight: inherit; text-align: var(--text-align); background-color: var(--ast-global-color-5);\">La participation active du patient est une condition n\u00e9cessaire au succ\u00e8s de ce PET-HTA.<\/span><\/p><p>Le suivi et la coordination permanente de ce PET-HTA est assur\u00e9 par le r\u00e9seau HTA-GWAD, qui sera aid\u00e9 pour cela par des \u00ab r\u00e9f\u00e9rents \u00bb r\u00e9partis sur le territoire de toute de la Guadeloupe.<\/p><p style=\"font-size: 15px; font-style: normal; font-weight: 300;\">Ces r\u00e9f\u00e9rents sont des professionnels de sant\u00e9, (m\u00e9decins, infirmiers, pharmaciens, di\u00e9t\u00e9ticiens, psychologues, aides \u00e9ducateurs sportifs ) qui ont sign\u00e9 une convention de partenariat avec le r\u00e9seau.<\/p><p style=\"font-size: 15px; font-style: normal; font-weight: 300;\">\u00a0<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-e9401a9 e-flex e-con-boxed e-con e-parent\" data-id=\"e9401a9\" data-element_type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-235be07 elementor-widget elementor-widget-text-editor\" data-id=\"235be07\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p><span style=\"font-style: inherit; font-weight: inherit; text-align: var(--text-align); background-color: var(--ast-global-color-5);\">Ces \u00ab r\u00e9f\u00e9rents \u00bb qui sont des acteurs essentiels de la mise en place et du suivi de ce PET-HTA ont \u00e9t\u00e9 form\u00e9s \u00e0 la probl\u00e9matique de l\u2019HTA connaissent ce programme, gr\u00e2ce \u00e0 une formation assur\u00e9e par le r\u00e9seau ont acquis des comp\u00e9tences en \u00e9ducation th\u00e9rapeutique, (au minimum le niveau 2 propos\u00e9 par l\u2019HAS en 2007).<\/span><\/p><p>Le triumvirat : \u00ab M\u00e9decin traitant \u2013 IDE \u2013 R\u00e9seau \u00bb, sera en permanence mobilis\u00e9 et en contact afin de d\u00e9velopper ce PET-HTA, avec comme \u00ab acteur \u00bb central le patient hypertendu.<\/p><p>Le M\u00e9decin traitant du patient sera syst\u00e9matiquement inform\u00e9 et associ\u00e9 aux diff\u00e9rentes \u00e9tapes de ce PET-HTA (par le biais d\u2019une fiche de circulation papier ou via le DMPI par Internet).
